Field Description
What is your current employment status? (Drop-Down) Select from the following options:
  • Not employed
  • Employed
  • Employed, but with notice of termination or layoff
  • Not in the labor force (not actively seeking work)
  • Someone who is “not in the labor force” might be someone who does not have a job and is not looking for a job. They may be using the system to find education and training.
How many total weeks (if any) have you been unemployed in the past 12 months? (Drop-Down) Select a number of weeks from 1 through 26 or one of the other options:
  • N/A
  • More than 26 weeks
  • Less than 1 week
Are you employed at less than full time, or at jobs inadequate with respect to your training or economic needs (Underemployed Worker)? (Drop-Down) Select No or Yes.
Have you been unemployed for 27 non-consecutive weeks in the past 12 months? (Drop-Down) Select No or Yes.
Have you been unemployed for 27 or more consecutive weeks at the time of registration? (Drop-Down) Select No or Yes.
