Work Eligibility

The following instructions apply to the Work Eligibility section of the Demographics Information. For this page, the type of field is identified, as well as the description for each field.

1. Work Eligibility Status: Drop-down; select from one of the following options:
   • U.S. Citizen
   • Non-Citizen Eligible to Work in the U.S.
   • Non-Citizen Not Eligible to Work in the U.S.
   • Refugee: Drop-down; select Yes, No, or Not Reported
2. USCIS Form Number: Drop-down; Select from the following options:
   • USCIS I-94
   • USCIS I-179
   • USCIS I-197
   • USCIS I-551
   • USCIS I-688
   • USCIS I-688A
   • USCIS I-688B
   • USCIS I-766
   • FS-545
   • DS-1350
   • Other
3. Alien Certification Number: Text; enter the client's number. Required for selecting a USCIS form, except Other.
4. Other Description: Text; enter a description of the USCIS Form Number not listed. This field is required if Other is selected in the USCIS Form Number drop-down.
