Dislocated Worker
The following instructions apply to the Dislocated Worker section of the Demographics Information (Page 1, Previous Occupation, Statewide 15%, Layoff Details, and HGEI). For the first page, the list of options is provided; for the additional pages, the type of field is identified, as well as the description for each field. Refer to Step 2: Add Client Record for more information about when pages display.
Dislocated Worker (Page 1)
Please select the ONE that best describes your situation:
- Have you been laid off or received a notice of layoff from your employer as a result of a reduction in the employer's workforce or received a notice of termination from your employer?
- Have you been laid off or received a notice of layoff from your employer as a result of a permanent closing or major layoff?
- Are you employed by an employer who has a made a general announcement that the business will close within 180 days?
- Are you employed by an employer who has made a general announcement that the business will close, without naming a specific date?
- Were you self-employed and are now unemployed due to general economic conditions or a natural disaster in your community?
- Are you a displaced homemaker? A displaced homemaker is someone who has been dependent on the income of another family member but is no longer supported by that income or is the dependent spouse of an active duty service member and the family income is significantly reduced because of a deployment, call or order to active duty, a permanent change of duty station, or the service-connected death or disability of that military member; and is unemployed or underemployed and experiencing difficulty in obtaining or upgrading employment.
- Are you the spouse of a military service member on active duty, and have you experienced a loss of employment as the direct result of a relocation to accommodate a permanent change in duty station of that service member?
- Are you the spouse of a military service member on active duty, and are you unemployed or underemployed and experiencing difficulty in obtaining or upgrading employment?
- Are you unemployed as a result of military closures or realignments?
- Were you a member of the Armed Forces, or a spouse of a member of the Armed Forces on active duty, seeking employment and training activities in an area where demand for these services exceeds the State and local resources' ability to provide these services?
- Are you unemployed as a result of an emergency or disaster situation of national significance that could result in a potentially large loss of employment, as declared or otherwise recognized by the chief official of a Federal Agency with jurisdiction over the Federal response to the emergency or disaster situation?
- Are you unemployed due to a situation where a substantial number of workers from a State, tribal area, or outlying area in which an emergency or disaster has occurred relocate to another State, tribal area, or outlying area?
- Are you unemployed due to multiple layoffs in a single community, significantly increasing the total number of unemployed workers?
- Are you unemployed due to emergencies or natural disasters which have been declared eligible for public assistance by the Federal Emergency Management Agency (FEMA)?
- None of the above.
If Question 1 is selected, the Previous Occupation page displays. If Questions 2-14 are selected, the Layoff Details page displays. If Question 15 is selected, the KHPOP (if applicable) and R/ATAA page display.
Dislocated Worker (Previous Occupation)

- Have you received information that you are eligible for unemployment benefits or have exhausted benefits? Drop-down; Select Yes or No.
- Have you received information that you are not eligible for unemployment benefits due to lack of sufficient earnings or that you performed services for an employer not covered by unemployment insurance? Drop-down; Select Yes or No.
If Questions 2 or 3 are Yes, the Layoff Details page displays.
Dislocated Worker (Statewide 15% Program)
Statewide 15% Program ID: Select a Statewide 15% enrollment from the drop-down list. Varies by state.
Dislocated Worker (Layoff Details)
- Use the drop-down to locate your layoff employer: Drop-down; select layoff employer if listed.
- If the employer is not listed in the drop-down, use the Employer Lookup button to search for the employer: Button; click the Employer Lookup button. If employer is not found, complete the termination or layoff date.
- Please enter the termination or layoff date: Text/calendar; enter the date of termination or layoff in mm/dd/yyyy format.
- Rapid Response Event Number: Text; enter the client's RRE number.
- Industry of dislocation (NAICS code): Display/button; click the NAICS Lookup button to select the industry from which the client was laid off.
- Occupation of dislocation (NAICS code): Display/button; click the NAICS Lookup button to select the occupation from which the client was laid off.
- Industry of dislocation (O*NET code): Display/button; click the ONET Lookup button to select the industry from which the client was laid off.
- Occupation of dislocation (O*NET code): Display/button; click the ONET Lookup button to select the occupation from which the client was laid off.
- Hourly wage at dislocation: Text; enter the hourly wage of the dislocation job.
- Hours per week at dislocation: Text; enter the number of hours worked per week at the dislocation job.
- Number of months at employer of dislocation: Text; enter the number of months worked at the dislocation job.
- Approved TAA Petition Number: Drop-down; select the petition number if client is covered by a Trade Assistance Act petition.
- Approved NAFTA Petition Number: Text; enter the petition number if the client is covered by a North Atlantic Free Trade Act.
- Approved TRA Claim: Text; enter the client's approved TRA claim number.
- NEG Enrollment 1, 2, or 3: Drop-down; select the appropriate grant to enroll in a National Employment Grant.
- Statewide 15% Program ID: Drop-down; select the enrollment to enroll in a program funded with 15% set aside funds.
