Personal Information

The following instructions apply to the personal information (Staff-Assisted Job Seeker Account Creation and Registration Information pages) of the Demographics Information. The type of field is identified, as well as the description for each field. For high-level description of creating a client account, please see Add Client Overview.

Staff-Assisted Job Seeker Account Creation

  • Social Security Number: Text; Enter the client's Social Security number.
  • Confirm Social Security Number: Text; Confirm the client's SSN. If they do not wish to provider their SSN, check the "I decline the entry of my Social Security number" box.
  • Date of Birth: Text/Date picker; Enter the client's date of birth (yyyy-mm-dd).

Account Information

  • Username: Text; Enter a unique and memorable username. The username must be between 6 and 20 characters.
  • Confirm Username: Text; Re-enter the username.
  • Password: Text; Must be 8 to 20 characters, include letters, at least one number, and a special character. Passwords are case sensitive.
    • Staff-assisted job seekers will be prompted to reset their password upon their first login to JobLink.
  • Confirm Password: Text; Re-enter the password.
  • Security Question: Text; Select a question from the drop-down.
  • Answer: Text; Enter answer to question selected.
  • Forgot Username or Password: Link; If the username entered displays as already taken, click the link to retrieve the password.

Profile Information

  • First Name: Text; Enter the client's first name.
  • Middle Initial: Text; Enter the client's middle initial.
  • Last Name: Text; Enter the client's last name.
  • Phone: Text; Enter the client's telephone number. If the phone number has an extension, enter as ###-###-####x###.
  • Cell Phone: Text; Enter the client's cell phone.
  • Fax: Text; Enter the client's fax number.
  • Email Address: Text; Enter the client's email address.An email will be sent for the job seeker to confirm they have access to that account.
  • Confirm Email Address: Text; Re-enter the client's email address.
  • Address: Text; Enter the client's street address.
  • Address 2: Text; Enter additional information for the client's address, such as apartment number.
  • City: Text; Enter the client's city of residence.
  • State: Drop-down; Select the client's state of residence.
  • ZIP Code: Text; Enter the client's ZIP/postal code.
  • Country: Drop-down; Default is the United States; select another country for international clients.
  • International Locality: Text; Enter the state/province/county for international clients.
  • Work Email: Text; Enter the client's work email address.
  • Work Phone: Text; Enter the client's work phone number.
  • How did you hear about our services? (Check all that apply.) Checkboxes; Select from the following options:
    • Word of mouth
    • Brochure/Flyer
    • Internet
    • Newspaper
    • Radio
    • TV
    • Highway Billboard
    • Other (text box)

Registration Information

  • Social Security Number: Text; If the client's SSN was entered on the Staff-Assisted Job Seeker Account Creation page, the SSN is prefilled.
  • Decline Entry of SSN: Check box; If the client declines to enter his/her SSN, AJL generates a pseudo SSN. These pseudo SSN accounts are not included in performance measures; AJL omits.
  • Date of Birth: Text or calendar; the client's date of birth is pre-filled.
  • Ethnic Hispanic or Latino: Drop-down; : A person of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture in origin, regardless of race. Select Yes, No, or Not Self-Identified. If No or Not Self-Identified is selected, Race is required. The client is not required to identify a specific race, but a response to the question is required.
  • Race: Check box; Check all that apply:
    • Asian or Asian American: A person having origins in any of the original peoples of the Far East, Southeast Asia, or the Indian Subcontinent (e.g., India, Pakistan, Bangladesh, Sri Lanka, Nepal, Sikkim, and Bhutan). This area includes, for example, Cambodia, China, Japan, Korea, Malaysia, he Philippine Islands, Thailand, and Vietnam.
    • Black or African American: A person having origins in any of the black racial groups of Africa.
    • Hawaiian or Other Pacific Islander: A person having origins in any of the original peoples of Hawaii, Guam, Samoa, or other Pacific Islands.
    • American Indian or Eskimo: A person having origins in any of the original peoples of North America and South America, including Central America, and who maintains their cultural identification through tribal affiliation or community recognition.
    • More Than One Race: A person who identifies as more than one race. The specific races with which the client identifies may be checked or may not be checked.
    • Not Self-Identified: A person who chooses not to identify race.
  • English Language Learner: Radio; Select Yes if English is NOT the client's primary language; select No if the client's primary language is English.
  • ELL Primary Language: Drop-down; Select the client's primary language from the list provided. If the client's primary language is not listed, select Other.
  • ELL Primary Language (If Other selected above): Text; Type the client's primary language if the language did not appear in the drop-down list above.
  • Do you prefer to communicate in sign language? Drop-down; Select No, American Sign Language, or Other Sign Language.
  • Sex: Drop-down; Select Male, Female, or Prefer not to answer.
  • Are you a single, separated, divorced or widowed individual with primary responsibility for one or more dependents under the age 18 (including single pregnant women): Drop-down; Select Yes; Yes, Head of Household; No; No, Head of Household; Not Reported.
  • Number in Family (Including Yourself): Text; Enter the number in client's family, including the client.
  • Individual with Disability (For this question, disability means a physical or mental impairment that substantially limits one or more of the major life activities of the individual.) Drop-down; Select Yes, No, or Prefer Not to Answer.
  • Are you a registered apprentice? Drop-down; select Yes, No, or Unknown.
  • Have you served on Active Duty with the Armed Forces of the United States? Drop-down; Select Yes or No.
